What learning can summaries add to reviews?

At present, the Learning from Adverse Events Team is working on developing a robust means of sharing learning from adverse events. Previous modes of learning are not conducive to supporting sharing learning, particularly in the current post-pandemic climate. ECHO incorporates the use of technology to support sharing of learning from adverse events and widens the reach to all health boards within Scotland.

Members of the Adverse Events Community of Practice are well engaged and have highlighted their desire to share learning of adverse events. A series of four ECHO’s will allow specific adverse event themes to be explored.

We have determined the theme for the first ECHO as suicide with the following rationale:

  • Aligns with Scot Govt Suicide Prevention Strategy due to be published in September/October 2022. Also aligns with Suicide Prevention Day on October 10th
  • Due to suicides multi-factorial nature, it can be complex to undertake learning from a completed suicide.

We have suggested the following topics for the remaining three. These were discussed with the AE Network on Tuesday 30 Aug and the response was very positive.

  • Pressure ulcers
  • Falls
  • Violence and aggression.

These topics have been suggested as have been recent topics of discussion at network meetings and on the Teams network discussion forum. These themes are also nuanced, complex topics different variations in how these events are categorised and reviewed.

Aim

To give those involved in learning from adverse events the opportunity to unpick challenging and complex scenarios and cases in a safe and professional environment.

To create or draft a new version of the learning summary which is conducive to sharing learning on that specific topic/theme.

Who is it for?

This echo network is for clinicians involved in reviews risk/governance leads, adverse event reviewers. We are interested in hearing from staff across all areas of the health service with an interest in how involving patients and families in reviews enhances learning.
